How to Make a Referral to a

Community Learning Disability Team.

 

 

To make a referral to a Community Learning Disability Team, please address a letter to the Team Co-ordinator of the Community Learning Disability Team that covers your area.  The address of each team appears below.  Alternately, you may call the Administrative Secretary for the Team in your area, and request a referral form.

 

 

What to include in your referral letter.

 

·       The client’s full name.

·       The client's date of birth.

·       The client's home address.

·       The client's primary day placement address.

·       Names of contact persons at the day placement and residence.

·       The name and address of the client's GP.

·       A brief statement of the referral issue.

·       Background information relevant to the referral issue.
